Listed below are the eligibility criteria for award candidates. A separate application form should be completed for each nomination (forms are available below).
Two awards may be given each year for outstanding and meritorious service in the field of Public Health, one of which shall be given to a person employed in a health-related field and one given to a person in a field not related to a health profession.
The work of the individuals chosen shall have made an outstanding contribution to the health of the population.
Potential recipients should qualify in at least one of the following categories:
The honor shall not be restricted to long tenure, or advanced age; but if given to a young individual, the merit of the contribution must be such that it cannot be questioned.

Candidates for the honor shall be considered each year, and the awards made only if justified.
